Script non funzionante informativa sulla privacy

Ciao a tutti.
Stò cercando di risolvere un problema mi spiego:
Ho un form in php che raccoglie ed invia dei dati, ho la necessita di costringere l'utente ad accettare l'informativa sulla privacy. Ho trovato uno script in java che fà al caso mio ma non riesco a farlo funzionare. Qualcuno mi può aiutare?

Il codice del submit è questo:

La checkbox:

il codice java utilizzato:

Il nome della pagina (che a sua volta viene caricata in una pagina .html) è prova.php

Il nome del form dovrebbe essere modulo.

Non puoi usare form come nome di variabile... e' una parola riservata

Affermi che il nome del tuo form e' un altro, perche' non l'hai sostituito nello script?

Il resto non l'ho guardato

ps: non postare il sorgente PHP: al browser arriva qualcosa che dipende dalle variabili in gioco sul server, postaci cio' che arriva al browser.

Si scusa quello è diciamo il master, nel codice sopra ho inserito modulo
form=document.modulo //ci metti il nome del tuo form

Scusa ma no capisco cosa intendi non sono molto pratico ne di php ne di java e infatti mi sono ritrovato fra le mani stò problema che mi stà facendo uscire pazzo.

Se tu verifichi sul browser (menu visualizza->HTML o visualizza sorgente pagina o ...) non vedrai mai istruzioni PHP... ecco, nemmeno noi
