Penso sia la struttura delle directory ad essere sbagliata.

Prova con:

->test (nome applicazione web)
--->>WEB-INF
------->>>classes
------->>>lib
------->>>web.xml
--->>images
------->>>PAR.jpg
--->>SimpleCaptcha.html


Cambiando opportunamente il mapping in web.xml le cose dovrebbero andare...
