tratta la stringa con strtolower, dopodichè usa ucfirst

$var = ucfirst(strtolower('mIa StrInGA')); 
per stringhe utf8 e simili:
purtroppo non c'è una ucfirst che tratta stringhe multibyte, per cui ti devi arrangiare (es: http://stackoverflow.com/questions/2...cter-encodings )
