Problema di logica in PHP

Dunque sto cercando di stampare su file PDF (sto inziando... perchè poi sarà più corposa e complessa) ed ho trovato difficoltà nell'uso du uno script che ho 'gentilmente' preso da FPDF.ORG, l'ho adattato alla mia esigenza e non riesco a far stampare la riga di intestazione.

Potete dirmi, credo sia un problema di logica di programmazione PHP, dove lo script non esegue oppure manca qualcosa che inavvertitamente non riesco proprio ad individuare.

Sto eseguendo la stampa di una lista.
